NEW CLIENTS

Thank you for entrusting your dog’s wellness to us.  This is a responsibility we take very seriously and consider your trust in us as one of the biggest privileges to earn.

For your understanding of the process, we have broken it down into six steps, most of which we initiate with e-mailed forms. Most importantly, we want you to be assured that Dr. Patnode will be coming to you to evaluate and treat your dog. Since every patient is unique, the treatment will vary depending on examination findings and treatment plan for your dog. This will be determined after the Inital Evaluation that Dr. Patnode will perform.

The process will be simple once you set up your appointment:

    • We will send you the form(s) necessary to get all the information we need to get your dog evaluated and treated!
    • Everything will be digital, there is no paper to print, mail or deal with.
    • We have streamlined the documentation for you for ease of use and efficiency of the process. We have laid out this process in 6 simple steps below, just to help you keep track of what is needed. But don’t worry, we will help you through every step!
      • Step 6 is an  introduction to our Pet Portal which we provide to you for access to your dog’s information portal, communication and appointment management.

New Client Form (Welcome to Our Practice!)

STEP 1

This form will be emailed to you upon setting up your dog’s first appointment. This form will assemble all of your information as well as your dog’s general information (complete medical history will be obtained from regular veterinarian).

We make it easy to complete this online form. You can do this from your home computer, tablet or your smart phone! No paper forms will be needed.

If you don’t receive this form within the same business day that you contacted us to make your appointment, please contact us by emailing admin@vrmohio.com -or- texting/calling 740-VRM-OHIO. We will re-send this to you!

Pre-Exam Questionnaire

STEP 2

This form will be e-mailed to you right after your appointment is set up. This questionnaire will cover the details of your dog’s current problem as well as get more information about them (diet, medications, supplements, behavior needs, etc.) If you don’t get this within a week of your upcoming appointment, please reach out to us at admin@vrmohio.com -or- texting/calling 740-VRM-OHIO and we will send you another copy right away! 
For your convenience, this form will be able to be completed on your home computer, tablet or smart phone! 

Evaluation and Treatment Consent Form

STEP 3

This is a form that will be required for each evaluation or treatment that will be done by Dr. Patnode. This will come to you as a link in an email from us.  If you don’t receive this by the day before your appointment, please contact us by emailing admin@vrmohio.com -or- texting/calling 740-VRM-OHIO. We can easily re-send this to you to complete. This will also be an online form – no paper forms needed.

Imaging & Radiologist Reports

STEP 4

If Dr. Patnode has requested any lab work, imaging or diganostics from other veterinarians or specialists, please be sure to contact those practitioners and have the specified documents sent to us at: admin@vrmohio.com

We will let you know if we need further results and diagnostic imaging.

Gait Videos

STEP 5

If Dr. Patnode has requested videos of your dog running/walking/doing their activity, please record on your cell phone and email to: admin@vrmohio.com

We will let you know if we need further recordings.

Pet Portal

STEP 6

We provide you a secure and absolutely free Pet Portal where you can sign in and manage your dog’s care. From requesting appointments to communicating with our team, you can do it all from our Pet Portal. Click HERE to be directed to the Portal.
